Responsibilities

• Actively participates in population health outreach and closes gaps in care.
• Perform pre-visit planning and data reviews before patient visits.
• Assist providers with care gaps and activation of standing orders.
• Document clinical encounters under supervision and support order entry.
• Coach patients using motivational interviewing to support self-management.
• Review registry and quality data to support population health goals.
• Identify high-risk patients and coordinate care management referrals.
• Collaborate with care team to reduce unnecessary ED visits.
• Maintain and stock treatment areas and sterilize instruments.
• Support in-house testing per standards.
• Perform additional duties as required.
