Job summary

Allegheny Health Network is seeking a Medical Assistant to support population health management, assist clinicians, and provide self-management coaching to patients.

You will participate in patient visits, review charts, and help with data entry, with emphasis on care coordination, compliance, and ensuring smooth clinic operations. Prior MA experience and PA credentials are preferred.

Qualifications

• Minimum MA credentials and 2 years of MA experience.
• State license or certification as described in qualifications.
• CPR certification required.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care using registries.
• Performs pre-visit planning by chart and data reviews before patient visits.
• Assists provider in addressing care gaps and activates standing orders.
• Documents encounter elements under supervision and assists with order entry.
• Coaches patients toward self-management using motivational interviewing.
• Performs registry and quality data review to close gaps in care.
• Identifies high-risk patients and coordinates complex care referrals with the care team.
• Assesses patient access to care and works to reduce avoidable ED visits.
• Ensures patient treatment areas are stocked and instruments sterilized.
• Performs in-house testing following established standards.
