Qualifications

• Minimum High School / GED or Completion of a Medical Assistant Program or LPN License Current State licensure OR Medical Assistant Certification.
• 2 years of Medical Assistant experience preferred.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care and facilitates closing gaps in care.
• Ensures pre-visit planning by chart and data review and prepares for patient arrival.
• Assists provider with expanded rooming protocol and activation of standing orders.
• Accurately performs clinical transcription and assists with order entry under supervision.
• Coaches patients toward self-management using motivational interviewing.
• Performs registry and quality data review and conducts population outreach.
• Oversees identification of high-risk patients and care coordination referrals.
• Reviews with care team to achieve population health goals and improve access to care.
• Cleans and sterilizes instruments and ensures inventory of medical supplies.
• Performs in-house testing and other duties as assigned.
