OnePeak Medical is seeking a passionate Nurse Practitioner to join our primary care team in The Dalles, OR. You will provide comprehensive medical services, perform assessments, order labs, and develop personalized treatment plans within a supportive, collaborative care model.

• Active Oregon NP license or eligible to obtain one.
• Board certification in Family or Internal Medicine preferred.
• Clinical practice experience, especially in functional/integrative medicine.

Responsibilities

• Provide direct patient care via office visits and telehealth.
• Order and interpret labs/imaging and diagnose conditions.
• Develop personalized treatment plans including lifestyle interventions and therapies.
• Educate patients on preventive and functional medicine concepts.
• Collaborate with health coaches, nutritionists, and support staff.
• Document visits in NextGen EHR accurately and timely.
• Participate in team meetings and quality improvement initiatives.
• Contribute to a positive clinic environment and operations.

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
