Job summary

OnePeak Medical seeks a Nurse Practitioner to join our Primary Care Functional Medicine team in Talent, Oregon. You will deliver comprehensive medical services, conduct office visits and telehealth, and collaborate with a high-functioning care team to drive root-cause health outcomes.

Relocation offered, no nights or weekends, a four-day work week, strong clinical support, mentorship, and comprehensive benefits.

Qualifications

• Active Oregon NP license or eligible for licensure.
• Board certification in Family Medicine or Internal Medicine.
• Strong interest in preventive and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Experience with NextGen EHR is a plus.

Responsibilities

• Provide direct patient care via office visits and telehealth.
• Perform comprehensive assessments and diagnose conditions.
• Develop personalized treatment plans including lifestyle interventions.
• Educate patients on preventive health and functional medicine.
• Collaborate with health coaches, nutritionists, and mental health staff.

Compensation: Guaranteed salary with earning potential of $300k+, relocation offered

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO (or eligibility to obtain )
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
