Job summary

OnePeak Medical is seeking a Medical Provider to join our growing team in Oregon. This role focuses on whole-person care, delivering comprehensive medical services, and building lasting patient relationships within a high-functioning care team.

You will provide direct patient care across office visits and telehealth, develop personalized treatment plans, and collaborate with health coaches and staff to drive optimal health outcomes.

Qualifications

• Active Oregon license as NP/PA/MD/DO (or eligible)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

RESPONSIBILITIES AND DUTIES
