Job summary

OnePeak Medical in Oregon is seeking a licensed medical provider to join a forward‑thinking team delivering whole‑person care. You will conduct office visits and telehealth, develop personalized treatment plans, and collaborate with a multidisciplinary care team to optimize patient health.

Qualifications

• Active Oregon license (NP/PA/MD/DO) or eligibility.
• Board certification in Family or Internal Medicine.
• Minimum 2 years clinical experience; functional/integrative preferred.
• Strong interest in preventive, functional, and lifestyle medicine.
• Excellent communication and patient relationship skills.
• Experience with NextGen EHR is a plus.
• Ability to work in a fast-paced, collaborative environment.

Responsibilities

• Provide direct patient care through office visits and telehealth.
• Perform comprehensive assessments; order labs/imaging; diagnose acute/chronic conditions.
• Develop personalized treatment plans including lifestyle interventions, supplements, hormone therapies, and prescriptions.
• Educate patients on preventive health and functional medicine principles.
• Collaborate with health coaches, nutritionists, mental health providers, and clinical staff.
• Document visits in the NextGen EHR in a timely, compliant manner.
• Participate in team meetings, quality improvement initiatives, and training sessions.
• Support clinic operations by contributing to a positive, team-oriented environment.

RESPONSIBILITIES AND DUTIES

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner*, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
