OnePeak Medical is seeking a Nurse Practitioner for Primary Care Functional Medicine in Oregon. You will provide direct patient care through office visits and telehealth, developing personalized treatment plans that may include lifestyle interventions and hormone therapies.

Qualifications

• Active Oregon NP license or eligibility
• Board certification in Family/Internal Medicine or applicable specialty
• Minimum clinical experience with emphasis on preventive/functional medicine
• Strong communication and patient relationship skills
• Proficient in EHR documentation and care coordination

Responsibilities

• Provide direct patient care via office visits and telehealth
• Perform comprehensive assessments and interpret labs/imaging
• Develop personalized treatment plans including lifestyle interventions and prescriptions
• Educate patients on preventive and functional medicine principles
• Collaborate with health coaches, nutritionists, and mental health providers
• Document visits in NextGen accurately and timely
• Participate in team meetings and quality improvement initiatives

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO ( or eligibility to obtain )
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Minimum 2 years of clinical experience preferred (functional/integrative medicine experience is a plus)
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient in using electronic health records (NextGen experience is a plus)
