OnePeak Medical in Roseburg, OR, is seeking a passionate Nurse Practitioner to deliver comprehensive primary care within a functional medicine framework. You will conduct office visits and telehealth, order labs, interpret results, and develop personalized treatment plans in collaboration with a high-functioning care team.

This role offers a guaranteed salary with earning potential of $300k+ plus relocation assistance, a 4-day work week with no nights or weekends, and strong clinical support

Qualifications

• Oregon NP license active or eligible.
• Board certification in Family or Internal Medicine.
• Excellent communication and patient relationship skills.

Responsibilities

• Provide direct patient care through office visits and telehealth.
• Order and interpret labs/imaging; diagnose acute and chronic conditions.
• Develop personalized treatment plans including lifestyle interventions and prescriptions.
• Educate patients on preventive and functional medicine principles.
• Collaborate with health coaches, nutritionists, mental health providers, and clinical staff.
• Document visits in the EHR timely and compliant.
• Participate in team meetings and quality improvement initiatives.
• Support clinic operations with a positive team environment.

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
