Qualifications

• Active Oregon license as NP, PA-C, MD, or DO or eligibility
• Board certification in Family or Internal Medicine or applicable specialty
• Strong interest in preventive, functional and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable in fast-paced, collaborative environment
• Proficient in using electronic health records

Responsibilities

• Provide direct patient care through office visits and telehealth
• Perform comprehensive assessments and diagnose conditions
• Develop personalized treatment plans including lifestyle interventions and prescriptions
• Educate patients on preventive health and functional medicine
• Collaborate with health coaches, nutritionists, mental health providers
• Document visits accurately in the EHR and participate in team meetings

Compensation: Guaranteed salary with earning potential of $300k+, relocation offered

RESPONSIBILITIES AND DUTIES:

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment

REQUIRED QUALIFICATIONS

• Active Oregon license as a Nurse Practitioner *, PA-C, MD, or DO (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or applicable specialty
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
