Job summary

Outreach Community Health Centers, Inc. is seeking a Behavioral Health Consultant to join the primary care team under the Director of Behavioral Health Services.

The role focuses on identifying and triaging patients with medical and behavioral health problems, delivering psychoeducation, and developing individualized behavioral change plans. You will collaborate with clinicians, monitor patient progress, perform timely documentation, and coordinate care with external facilities.

Qualifications

• Master’s degree in Social Work or related field as required.
• Must hold or be eligible for Wisconsin social work licensure.

Responsibilities

• Assess behavioral and mental health status for referrals from primary care.
• Provide psychoeducation and develop behavioral change plans.
• Coordinate care with primary care team and external providers.
• Refer cases to specialty mental health services as appropriate.
• Document patient care efficiently and timely.
• Participate in meetings and quality improvement activities.
