Job summary

Heartland for Children in Bartow, Florida, is seeking an experienced professional to manage child welfare and mental health referrals. Responsibilities include establishing relationships, processing assessments, and attending multi-disciplinary meetings.

Job description

Essential Functions:

• Establish and maintain positive working relationships with Heartland for Children internal and external customers.
• Knowledgeable of child welfare and case management.
• Knowledgeable of mental health, utilization management and DJJ processes.
• Knowledgeable of Comprehensive Behavioral Health Assessments (CBHA), mental health referrals and mental health services.
• Receive and process CBHA referrals that are submitted by our partners within required timeframes.
• Periodically review CBHAs for quality and provide feedback to providers when necessary.
• Provide connections and support for mental health, substance and behavioral health needs of children and adults.
• Attend DJJ lockout staffings, coordinate meetings and recommend services for kids to help divert placement in licensed care.
• Participate in multi-disciplinary staffings to include but not limited to Crossover Youth Staffings, Youth at Risk Staffings, Local Review Team Staffings and other staffings as requested.
• Identify trends, good practice and barriers related to these processes and make written recommendations for improvement and/or training opportunities.
• Perform additional duties as assigned.

Qualifications:

Bachelor’s degree in social services, health care, business or related field required. Master’s degree preferred. Extensive experience in child welfare and/or child abuse prevention preferred.
