Job summary

Heartland for Children seeks a Program Manager-Prevention & Diversion to oversee prevention and diversion support services for families, with a focus on connecting resources and reducing barriers for children and adults with mental health, substance use, and behavioral health needs.

Qualifications

  • Bachelor’s degree in Social Services, Health Care, Business or related field required.
  • Master’s degree preferred.
  • Minimum of three to five years of professional experience in the related field with supervisory experience.
  • Extensive experience in child welfare and/or child abuse prevention preferred.
  • Excellent organization and follow up skills; able to multi-task; analytical ability; strong oral and written communication and customer service skills; proven leadership.

Responsibilities

  • Establishes and maintains positive working relationships with internal and external customers.
  • Supervises Family Support Specialists and resources for families to prevent entry into the child welfare system.
  • Ensures families access necessary resources timely and effectively to optimize outcomes.
  • Oversees development and delivery of resource navigation and support services to divert cases from entering the local child welfare system.
  • Coordinates and delivers community and staff presentations on prevention topics and service delivery.
  • Focuses on helping families before harm by removing barriers and identifying services.
  • Provide connections and support for mental health, substance and behavioral health needs of children and adults.
  • Supervises staff and case consultations to ensure family preservation.
  • Reviews data related to children entering the system and helps develop prevention strategies.
  • Reviews risk levels and determines interventions to support families.
  • Ensures documentation in FSFN and updates staff on case progress.
  • Models and enforces conflict resolution and open communication with staff and providers.
  • Responds urgently to family service needs and resources.
  • Attends meetings and coordinates activities related to prevention and diversion case oversight.
  • Participates in management meetings and special projects.

Program Manager-Prevention & Diversion

Applying For:

Job Summary: This is a management position responsible for the oversight of prevention and diversion support services for families, prevention education/awareness, identification of the least restrictive services, resources, and supports, with a goal of family preservation. This position will directly provide connections/support for children and adults with mental health, substance abuse and behavioral health needs.

Essential Functions:
  • Establishes and maintains positive working relationship with HFC internal/external customers
  • Manages and provides supervision to Family Support Specialists that are responsible for identifying resources and problem solving with families to prevent children from entering the child welfare system
  • Ensures families access the necessary resources timely, services and programs effectively, with the focus on optimizing their experience and outcomes
  • Oversee the development, implementation and delivery of resource navigation systems and support services to divert cases from further entering the local child welfare system in Circuit 10
  • Coordinates, creates, provides and delivers community and staff presentations on prevention related topics and service delivery
  • Focuses on providing families with help before harm, by removing barriers and identifying the most appropriate services and supports
  • Provide connections and support for mental health, substance and behavioral health needs of children and adults
  • Provides daily supervision of staff and case consultations on specific case situations to ensure family preservation
  • Reviews, compiles and tracks data related to children entering the system of care and helps to develop strategies to safely prevent entering the child welfare system
  • Reviews risk levels for families and determines interventions needed to support families
  • Ensure staff are documenting all efforts in FSFN and keeping all staff and teams updates on case progress
  • Models and enforces effective conflict resolution by encouraging open and free flowing communication between staff and service providers
  • Responds with a sense of urgency to identify family service needs and resources
  • Attends meetings and supervise/coordinate activities related to prevention and diversion case oversight
  • Participate in management meetings and management team projects
  • Performs special projects as assigned
Qualifications:

Bachelor’s degree in Social Services, Health Care, Business or related field required. Master’s degree preferred. Requires a minimum of three to five years of professional experience in the related field & supervisory experience. Extensive experience in child welfare and/or child abuse prevention preferred. Must have excellent organization and follow up skills, be process oriented, be able to multi-task, have good analytical ability, have good oral and written communication skills, customer service skills and proven leadership skills.
