Job summary

Qualifications

• Experience in care coordination and federal, state benefit enrollment.
• Excellent organizational skills.
• Creative and flexible within treatment program parameters.

Responsibilities

• Assess client needs, connect them to services, support Medicaid enrollment, coordinate care, document progress.
• Assess Medicaid status and follow agency policy to obtain/maintain status.
• Make referrals to fill gaps in assistance.
• Conduct screening, intake, assessment, housing plan development, and follow-up.
• Link clients to vocational, educational, housing, and social services.
• Deliver health education and community outreach through grant initiatives.
• Provide specialty support and prevention/harm reduction services.
• Ensure individualized care plans and maximize units of service.
• Use Motivational Interviewing to guide client decisions and resolve ambivalence.
• Understand programs and community resources; complete incident reports as required.
• Maintain HIPAA compliance and adhere to SAMHSA requirements.
