Job summary

Odyssey House Louisiana seeks a full-time Case Manager to work Monday–Friday, 8:30am–5:30pm. You will determine eligibility for guests to participate in the Agency’s Low Barrier Shelter and use Motivational Interviewing to assess needs and create individualized housing plans.

You will document progress, coordinate with community resources, assist with vital records, leases, and Medicaid enrollment, and ensure proper reporting to the Project Manager while supporting residents in securing stable

Qualifications

  • Valid driver’s license.
  • Excellent communication and organizational skills.
  • At least two years of case management experience.
  • Database management experience.
  • Ability to work with homeless guests of diverse backgrounds and disabilities including physical, mental, substance abuse, immunodeficiencies, etc.

Responsibilities

  • Provide case management to approximately thirty or more guests as assigned; Case management will involve screening, intake, assessment, individualized housing plan development, monitoring, linkage to appropriate community resources, follow up, appropriate transition to housing, etc.
  • Assist guests with obtaining vital records, IDs, Driver’s Licenses and other documentation.
  • Assist guests with completing lease agreements and obtaining other supportive documents.
  • Assess guests’ Medicaid status and follow agency policy to obtain and maintain Medicaid status.
  • Enter guests’ information into the agency’s database(s) in a timely manner and update as needed.
  • Prepare and submit reports to Project Manager in a timely manner.
  • Maintain up-to-date information regarding tenant occupancy, move outs, and vacancies.
  • Utilize Motivational Interviewing to elicit behavior change by guiding participants to explore and resolve ambivalence.
  • Ensure guests are referred to and/or aware of available vocational, educational, housing, and social services in the community.
  • Assist guests (either directly or via referral) in developing life skills to navigate environments, access resources, pursue opportunities, obtain housing, manage finances, and access public assistance.
  • Attend all required meetings and trainings.
  • Keep written progress notes of work done on the case.
  • Make necessary referrals to fill potential gaps in assistance.
  • Verify results of assistance provided to close the case per Program Manager approval.
  • Provide crisis management when indicated by guest needs.
  • Complete all Incident Reports prior to end of shift as required by Incident Policy and Procedure.
  • Maintain guests’ licensing requirements.
  • Oversee daily guest activities for guests in LBS; perform other duties as assigned.

Job Summary

Odyssey House Louisiana, Inc. is seeking a full-time Case Manager. The schedule is Monday- Friday 8:30am-5:30pm. The Case Manager position is responsible for determining eligibility for guests to participate in the Agency’s Low Barrier Shelter (LBS). The Case Manager will utilize Motivational Interviewing to assess the guest’s needs/disabilities to create an effective individualized housing plan.

  • assist guest(s) with obtaining vital records, ID cards, Driver’s Licenses and other documentation;
  • assist guest(s) with completing lease agreements and obtaining other supportive
  • documents such as identification, social security card, etc., as required by leasing agents/property;
  • assess guest(s) Medicaid status and follow agencies policy and procedures to obtain and maintain Medicaid status;
  • enter guest’s information into the appropriate agency’s database(s) in a timely matter and update as needed;
  • prepare and submit reports to Project Manager in a timely manner;
  • maintain up to date information regarding tenant occupancy, move outs, and vacancies
  • utilize Motivational Interviewing (MI), a client-centered approach for eliciting behavior change by guiding participants to explore and resolve ambivalence;
  • ensure that guest(s) are referred to and/or aware of available vocational, educational, housing, and social services in the community;
  • assist guest(s) (whether through direct intervention or referral to appropriate services) in the development of life skills necessary to: navigate the surrounding environment; access community resources such as banks, employment agencies, government offices, and recreational and educational organizations; serve as a resource to the community; pursue educational and occupational opportunities; obtain housing; manage finances; and access public assistance;
  • attend all required meetings and trainings;
  • keep written progress notes of work done on the case;
  • make necessary referrals to fill potential gaps in assistance;
  • verify the results of the assistance provided in order to close the case, as per the approval of the Program Manager;
  • provide crisis management when indicated by guest(s) needs;
  • complete all Incident Reports prior to the end of scheduled shift as required by Incident Policy and Procedure;
  • maintain guests’ licensing requirements;
  • oversee daily guest activities for guest(s) in the LBS; and
  • perform other duties as assigned.
