Job summary

CrescentCare is seeking a Social Worker/Social Services Case Manager in New Orleans to advance client engagement, assess eligibility, and link clients to medical and behavioral health care. The role focuses on reducing barriers and promoting self-management while coordinating across care systems.

Ideal candidates will hold a social work license or a nursing license in Louisiana, with relevant education and at least one year of post-degree HIV-focused direct service experience.

Qualifications

• Excellent verbal and written communication skills.
• Ability to complete complex projects.
• Experience with HIV-target populations is a plus.

Responsibilities

• Assess new and existing clients for program eligibility.
• Provide linkage to medical and behavioral health care.
• Document all interactions in the electronic health record (EHR).
• Advocate and assist clients with access to resources (food, housing, insurance).
• Coordinate primary care screening and monitoring of health indicators.
• Collaborate with treatment and recovery teams to integrate care.
