Job summary

Qualifications

• High School Diploma or equivalent. Work experience may NOT substitute for education requirements.
• At least 1 year of experience in social services, healthcare with a focus on assisting individuals with accessing healthcare resources and services.
• Knowledge of prescription assistance programs, pharmaceutical industry practices, and health care systems.

Responsibilities

• Serve as the primary point of contact for pharmaceutical companies and other organizations offering prescription assistance programs.
• Receive deliveries of medications from pharmaceutical companies, verify accuracy, and maintain records.
• Document all received medications in the EMR and notify patients of arrival.
• Identify patients in need of prescription assistance through screenings and referrals.
• Educate patients about available prescription programs, eligibility criteria, and procedures.
• Assist patients in completing application forms and submitting applications to programs.
• Advocate for patients to expedite processing and ensure access to medications.
• Coordinate enrollment with providers and patients and provide updates on status.
• Follow up with patients to track status and resolve barriers.
• Maintain accurate records of patients’ medication needs, applications, and outcomes in compliance with privacy regulations.
• Collaborate with case managers and healthcare professionals to address social determinants of health.
