Job summary

Community Healthcare Network is seeking a PrEP Navigator in New York, NY, to conduct outreach and inreach activities, assist with intakes, provide education to PEP/PrEP patients, and coordinate medical appointments. This role supports insurance troubleshooting, data entry into AIRS, and retention counseling.

The position requires a High School Diploma or GED, knowledge of HIV prevention, and strong communication skills.

Qualifications

• High School Diploma or G.E.D. equivalency.
• Basic knowledge of HIV prevention initiatives and PEP/PrEP.
• Good verbal and written communication skills.
• Knowledge of geographic area of work site and language appropriate to community.
• Proficient in Microsoft Word and Excel and data entry in AIRS.
• Respect for the rights of individuals to make their own decisions about their medical care.
• Ability to communicate easily and with a cordial manner toward diverse backgrounds.

Responsibilities

• Assess needs of patients seeking and accessing PEP/PrEP related services.
• Assist providers with identifying patients for PEP/PrEP and with intakes / enrollment, service navigation, education and follow up around PEP and PrEP services.
• Assist with data collection and data entry for AIRS and CHN’s EMR.
• Coordinate medical services with clinic staff.
• Provide outreach, education, and risk reduction interventions.
• Participate in health fairs and joint health activities with community agencies.
• Maintain confidentiality of patient information.
