Job summary

AltaMed in Anaheim, CA seeks a PrEP Navigator II for HIV/STI education, counseling and testing, and navigation to PrEP/PEP services. You will assist clients with ACA enrollment, scheduling, and access to Medication Assistance Programs.

Responsibilities include recruitment, intake, assessment, mentoring, referrals, and reporting progress toward program deliverables. Role involves collaboration with internal/external coalitions and team mentoring.

Qualifications

• Minimum 2 years of experience in a human services field and in HIV prevention or case management services.
• Experience delivering client-centered harm reduction counseling is highly desirable.
• Bilingual in English and Spanish preferred.

Responsibilities

• Refer clients for ACA enrollment and navigate clients onto PrEP/PEP services.
• Schedule medical appointments for PrEP/PEP and assist with Medication Assistance Programs.
• Conduct program recruitment, client intake and assessment, mentoring, and coordinate referrals.
• Monitor and report progress toward deliverables; represent AltaMed on internal/external workgroups and coalitions.
• Mentor other team members as part of Navigator II responsibilities.
