Job summary

AltaMed Health Services seeks a PrEP Navigator II in California to deliver HIV/STI education, counseling, testing, and navigation services to high-risk clients. You will coordinate referrals, enrollment, and appointments, and support with medication assistance programs.

You will work with a bilingual team, champion harm-reduction approaches, and help monitor program deliverables. A high school diploma is required; a Bachelor's in a health-related field is preferred, with 2 years in human

Qualifications

• High School Diploma required.
• Bachelor's Degree in social services or other health-related field preferred.
• Minimum 2 years of experience in a human services field, previous experience HIV prevention or case management preferred.
• Minimum 2 years of Motivational Interviewing or client-centered harm reduction counseling desirable.
• Experience delivering case management to low-income, at-risk populations preferred.
• Certification as HIV Test Counselor or eligible to obtain within 3 months.
• Valid BLS certification or higher following AHA/Red Cross guidelines.
• Bilingual, English and Spanish, preferred.
• Must be at least 21 years old.

Responsibilities

• The PrEP Navigator II provides comprehensive HIV/STI health education, HIV counseling and testing, and facilitates HIV Prevention and PrEP/PEP navigation services.
• Refers clients for ACA enrollment and navigates clients onto PrEP/PEP services.
• Schedules medical appointments for PrEP/PEP and assists with Medication Assistance Programs.
