Job summary

Colonial Group is seeking an experienced RN to assess, plan, implement and evaluate nursing care in alignment with organizational policies and professional standards. The role ensures quality care delivered by the nurse and team under their direction and emphasizes data-driven patient assessments and individualized age-appropriate care.

The position requires collaboration with the multidisciplinary team, timely documentation, and adherence to regulatory requirements.

Qualifications

• Associate degree or diploma in nursing is required.
• Current valid RN license in state of practice or compact licensure.
• BLS and ACLS certification required.
• PALS certification within 90 days if assigned ASC services pediatrics.

Responsibilities

• Assess physical, psychological, social, spiritual and educational needs and plan care.
• Formulate and adjust the plan of care with patient/family and multidisciplinary team.
• Implement care, coordinate, delegate and supervise the health care team.
• Document care plans, times, and patient responses in the medical record.
• Evaluate patient progress and effectiveness of teaching, adjust as needed.
• Contribute to evidence-based practice and mentor staff when appropriate.
• Provide care that aligns with policies, standards and regulatory requirements.
• Assist with discharge planning and transition of care.
• Provide high-quality service and promote continuous improvement.
