Job summary

Bloom Healthcare in Lakewood, CO is seeking a Practice Coordinator to manage administrative functions for a patient panel and care teams. You will schedule, coordinate visits, and communicate with physicians and staff to ensure timely referrals and records.

You will also build strong relationships with clinicians, leadership, patients and families, while driving performance metrics for productivity and patient satisfaction in a home-based care model.

Qualifications

• Excellent phone/customer service skills.
• Familiarity with EMR software a plus (eCW preferred).

Responsibilities

• Manage the administrative functions for a team of 500-600 patients and 3-5 physicians, nurse practitioners and physician’s assistants.
• Coordinate timely visits and demonstrate superior communication across care teams to meet homebound patient needs.
• Build trusted relationships with physicians, partner community leadership, patients and families, and Bloom senior leadership.
• Own team performance measures translating to provider productivity, patient satisfaction, and timely call answering.
• Balance competing demands including referrals, visits, records and supply requests with strong time management.
• Ensure patients are seen in a timely fashion as requested by providers and staff.
• Create efficient provider schedules to maximize driving route efficiency.
• Answer incoming phone calls and schedule appointments, including urgent slots.
• Process incoming faxes and provider requests for patient records.
• Order DME and oxygen as requested by providers.
• Send referrals to specialists, home health or hospice as requested by providers.
• Demonstrate superior communication across the care team.
