Job summary

Bloom Healthcare is seeking a Hospice Intake Coordinator to turn referrals into compassionate, compliant care for patients and families at the Lakewood, Colorado location.

You will review medical records, determine eligibility with physicians and providers, and guide families through what hospice means while answering questions and providing steady support throughout the decision process. You’ll ensure intake paperwork and consents are accurate and kept up to date.

Qualifications

• 1+ year of demonstrated customer service experience.
• 1+ year of experience with medical terminology.
• Advanced computer skills, including proficiency with EHR systems and the Microsoft Suite.
• Bachelor's degree.

Responsibilities

• Review medical records and coordinate with physicians and healthcare providers to determine eligibility.
• Guide patients and families through what hospice means and answer questions during decision-making.
• Ensure intake paperwork and consents are accurate and compliant.
• Coordinate with nurses, social workers and chaplains for a smooth transition into hospice care.
