Job summary

Sanitas is seeking a Case Management Coordinator to manage the transition of care for members post-discharge and those with chronic conditions. You will coordinate follow-up care, create personalized care plans, and educate patients and caregivers to prevent readmissions and improve health outcomes.

You will collaborate with primary care providers, specialists, home health agencies, and community resources to ensure seamless continuity of care across the care continuum.

Qualifications

• High school diploma or equivalent.
• Graduation from an MA program.
• Active MA certification including CPR/BLS if applicable.

Responsibilities

• Conduct timely post-discharge outreach within 24-72 hours and assess member needs.
• Coordinate follow-up appointments and services across care continuum.
• Develop individualized care plans and educate patients/caregivers to reduce readmissions.
• Document all interactions in EMR/care management platform with accuracy.
