Job summary

Sante Health System is seeking a Clinical Care Coordinator in the Utilization Management department. Requirements include an active RN or LVN license and a minimum of two years of nursing experience in ambulatory or hospital settings.

Responsibilities

• Develop and monitor care management plans for post-discharge patients across inpatient and community settings.
• Coordinate assessments and create realistic care plans with patients and PCPs.
• Apply nursing care management principles to ensure high-quality, cost-effective care.
• Maintain scheduled communication with PCPs, specialists, and care providers to review patient progress.
