Job summary

FOREST GROVE POST ACUTE in Forest Grove, OR is seeking a PRN Physical Therapist to evaluate and treat residents, collaborate with families and the healthcare team, and maintain accurate medical records.

You will develop individualized treatment plans to improve mobility and independence, supervise PT assistants, and participate in care conferences, always upholding infection control and safety standards.

Qualifications

• Bachelor's degree in Physical Therapy; licensure required.
• Proficiency with Net Health and Point Click Care software.
• Ability to read and interpret procedures and policies.
• Strong communication and teamwork with families and staff.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehab potential.
• Develop and implement individualized treatment plans to improve mobility.
• Supervise PT assistants and ensure documentation compliance.
• Communicate progress with physicians, nursing staff, and families.
• Maintain infection control and safety in therapy sessions.
