Job summary

PACS Group, Inc. Forest Grove Post Acute is seeking a Physical Therapist (PRN) to evaluate and treat residents, communicate with families and physicians, and maintain accurate medical records.

The role includes developing treatment plans to improve mobility, strength, balance, and functional independence while ensuring safety and compliance. The position requires state licensure, ability to supervise assistants, and collaboration with interdisciplinary teams.

Qualifications

• Education in Physical Therapy required; Master’s/Doctorate preferred.
• Proficient computer skills including Net Health and Point Click Care for documentation.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and communicate progress with families and physicians.
