Qualifications

• Current, valid California Physical Therapist (PT) license in good standing.
• New Graduates welcome; SNF experience a plus.

Responsibilities

• Perform high-quality PT evaluations and develop personalized treatment plans for post-acute and long-term care residents.
• Implement evidence-based treatments to improve mobility, balance, strength and functional independence.
• Collaborate with nursing, therapy, and medical teams to support resident recovery.
• Maintain precise and timely documentation per state and federal regulations.
• Educate residents and families on therapy programs, safety protocols, and long-term goals.
• Foster a positive, team-oriented environment focused on optimal resident outcomes.
