Qualifications
- Current California PT license in good standing.
- Strong communication and clinical reasoning in geriatric/post-acute settings.
- Experience in SNF/post-acute care is a plus; new grads welcome.
Responsibilities
- Perform PT evaluations and develop personalized treatment plans.
- Implement evidence-based treatments to improve mobility, balance, strength and independence.
- Collaborate with nursing, therapy and medical teams for holistic resident care.
- Maintain compliant documentation per state and federal requirements.
- Educate residents and families on therapy plans, safety, and goals.
- Foster a positive, team-focused clinical leadership environment.
