Qualifications

• Must be a licensed Certified Nurse Practitioner.
• Experience in post-acute or skilled nursing care is preferred.
• Strong communication and collaboration with care teams.

Responsibilities

• Diagnose, treat, and guide patients in a skilled nursing facility.
• Order and interpret diagnostic tests and imaging as needed.
• Prescribe medications and develop treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.
