Altea Healthcare is seeking a Certified Nurse Practitioner (NP) to join our post-acute care team as a key clinical provider in skilled nursing facilities. You’ll diagnose, treat, and guide patients toward better health, with a focus on geriatric and primary care needs.

Qualifications

• Certified NP with license or eligibility to obtain NP license.
• All experience levels welcome with training and mentorship.
• Passion for geriatrics, internal medicine, or primary care.
• Strong teamwork and excellent communication skills.
• Self-motivated with a patient-first approach.

Responsibilities

• Perform physical exams and review medical histories.
• Order and interpret diagnostic tests (labs, imaging).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.
