Job summary

Altea Healthcare is seeking a Certified Nurse Practitioner (NP) to lead clinical care in a post-acute skilled nursing facility. You will diagnose, treat, and guide patients toward improved health with a patient-first approach.

Qualifications

• Requires NP license or eligibility.
• Open to experienced providers and new graduates with training/mentorship available.
• Interest in geriatrics, internal medicine, or primary care.

Responsibilities

• Perform physical exams and review medical histories.
• Order and interpret diagnostic tests (labs, imaging).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses and facility staff.
• Educate patients and families on health conditions and preventive care.
• Document patient care accurately and efficiently.
