Qualifications

• Must hold an active NP license and CNM/NP certification.
• Minimum experience in post-acute or primary care is preferred.
• Strong clinical assessment and decision-making abilities.
• Excellent communication with patients, families, and team.

Responsibilities

• Perform physical exams and document medical histories.
• Order and interpret diagnostics (labs, imaging).
• Diagnose and manage acute and chronic conditions.
• Prescribe medications and create treatment plans.
• Collaborate with physicians, nurses, and facility staff.
• Educate patients and families on health and prevention.
• Maintain accurate and timely patient documentation.
