Qualifications

  • MD or DO degree required.
  • Current, unrestricted state medical license.
  • Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
  • Experience in post-acute care, skilled nursing facilities, or long-term care preferred.
  • Strong clinical judgment, communication, collaboration, and leadership skills.
  • Ability to work independently while partnering effectively with facility teams.

Responsibilities

  • Provide comprehensive medical care to residents in assigned SNFs.
  • Perform admissions, follow-up visits, acute visits, and regulatory visits.
  • Lead clinical decision-making to support chronic disease management and reduce avoidable hospitalizations.
  • Complete timely, accurate, and compliant documentation in the EMR.
  • Collaborate with nursing leadership, therapy teams, social services, administrative staff, and other interdisciplinary team members.
  • Participate in care conferences, IDT meetings, and quality improvement initiatives as needed.
  • Support facility performance related to Quality Measures, Five-Star ratings, readmission reduction, and survey readiness.
  • Communicate care plans and clinical updates to residents, families, facility staff, and external providers.
  • Participate in the on-call rotation as assigned.
  • Maintain professionalism and represent the organization with courtesy, discretion, and strong clinical leadership.

Post-Acute Care Physician Skilled Nursing Facilities
Position Summary

The Post-Acute Care Physician provides high-quality medical care to residents across assigned skilled nursing facilities. This role is responsible for managing a defined patient census, supporting facility leadership, and improving clinical outcomes, documentation, and operational performance. The physician will provide care for post-acute rehabilitation, long-term care, and custodial residents, including admissions, follow-up visits, acute visits, regulatory visits, and chronic disease management. This position requires strong clinical judgment, effective communication skills, and the ability to work independently while collaborating with facility teams.

  • Provide comprehensive medical care to residents in assigned skilled nursing facilities.

Schedule
  • Full-time.
  • In-facility presence required, with travel between assigned facilities as needed.
  • Flexible scheduling with autonomy over daily workflow.
  • Telehealth may supplement care when appropriate.
  • On-call rotation as assigned.
Compensation
  • Percentage-based compensation model with performance bonus opportunities.
  • 60/40 compensation model, with 60% of collected revenue paid to the physician.
  • Applies to billable services, including E/M visits, TCM, CCM, and procedures.
  • Weekly census bonus available when performance requirements are met.
  • Documentation performance incentives based on timely note completion, regulatory visit compliance, and coding accuracy.
  • Strong performers may have total annual earnings potential in the $300,000$400,000 range.
Requirements
