  • Conduct routine rounding, thorough evaluations, and timely follow‑ups to ensure continuity of care and early, aggressive intervention.

  • Lead and actively participate in multidisciplinary care team meetings; contribute to individualized care plans.

  • Aggressively manage chronic and complex conditions; respond promptly to status changes and emergent situations using appropriate modalities (e.g., IV diuretics, antibiotics, fluids, TPN) and respiratory therapies.

  • Communicate clearly and compassionately with nursing staff, NPs/APPs, patients, families, and consulting providers.

  • Collaborate with facility nursing, rehabilitation teams, leadership, and assigned NP to coordinate and optimize care.

  • Discuss goals of care and palliative care options; support patients and families in shared decision‑making.

  • Promote healthy aging through preventive care and proactive treatment plans.

  • Document accurately and promptly in the EMR; maintain strong clinical documentation habits.

  • Help develop and implement proactive strategies to reduce avoidable hospital transfers and readmissions.

Desired Qualities
  • Passion for patient‑centered care in geriatrics and chronic disease management.

  • Strong clinical judgment, confidence, and decisive decision‑making.

  • Excellent communication skills with patients, families, and interdisciplinary staff.

  • Proactive, assertive clinical management style; comfortable in fast‑paced, diverse clinical environments.

  • Highly organized with effective time management; detail‑oriented and efficient.

  • Independent and self‑directed; reliable, punctual, and hardworking.

  • Comfortable discussing sensitive topics, including goals of care and palliative needs.

Qualifications
