Job summary

Enhabit Home Health & Hospice is seeking a skilled referral relations specialist to drive growth with physicians, hospitals, and health systems. You will educate referral sources on our services and program requirements to support post-acute transitions and improve patient outcomes.

The ideal candidate has a Bachelor's degree in healthcare administration, marketing, or related field, plus at least two years in healthcare business development or sales.

Qualifications

• Bachelor's or graduate degree in healthcare/related field
• Minimum two years in healthcare business development, community relations, marketing, or sales
• Knowledge of Medicare home health and hospice regulations
• Valid driver's license and automobile liability insurance
• Strong interpersonal, communication, and presentation skills
• Experience with electronic referral systems (HCHB preferred)

Responsibilities

• Manage relationships between the company and referral sources such as physicians, hospitals, and health systems
• Grow referrals by educating sources on services and program requirements
• Help navigate post-acute care transitions to improve patient outcomes
• Support care transitions and ensure high patient/referral source satisfaction
