Job summary

Enhabit Home Health & Hospice is seeking a dedicated Referral Relations Manager to partner with physicians, hospitals, and allied providers to grow post-acute care referrals and ensure smooth transitions for patients.

The role emphasizes collaboration, knowledge of regulatory guidelines, and building long-term relationships; a bachelor's degree and healthcare business development experience are highly preferred.

Qualifications

• Bachelor's Degree in healthcare administration, marketing, or related field, or graduate of approved school of nursing, therapy, or social work (equivalent work experience accepted).
• Excellent interpersonal, communication, and presentation skills.
• Knowledge of federal, state, and local laws governing Medicare home health and hospice operations.
• Valid driver's license and automobile liability insurance.

Responsibilities

• Serve as primary relationship manager between Enhabit and referral sources (physicians, hospitals, senior living communities, etc.).
• Promote awareness of Enhabit services and program requirements to support referral growth and better patient outcomes.
• Navigate post-acute care transitions to ensure smooth care coordination and high satisfaction for patients and referral sources.
• Maintain relationships with healthcare organizations and partners to grow referral networks.
