Job Summary: The Population Health Coordinator, Community, is responsible for coordinating system-wide health initiatives within Englewood Health, The Shirvan Family Live Well Center (LWC) and the broader community. They are an integral member of the Population Health team and provide front-line support for the development and management of multiple programs. The Coordinator will be responsible for researching, coordinating, implementing, managing, and evaluating equity based wellness programs. They will be accountable for the day-to-day operations of these programs and must be proactive and possess the ability to multi-task, problem solve, with strong attention to detail. The Coordinator will be responsible for specific administrative tasks including knowledge for collecting and organizing data and providing data analysis and summary presentations. They will also support the Community Needs Assessment, as needed, as well as providing part time support to day-to-day operations of the LWC, also to include assistance in planning and coordinating program development, and coordination with local organizations.
