Job summary

Allegheny Health Network seeks a Medical Assistant to support population health outreach and CIN-based care coordination. The role involves assisting practices to engage patients with proactive outreach and coordinating care to meet health goals.

The position supports pre-visit planning, rooming, documentation, and order entry under supervision, using motivational interviewing to promote self-management and activation. Hybrid work is available.

Qualifications

• High School / GED or Medical Assistant Program or LPN License.
• Current PA LPN licensure or multi-state eNLC licensure or Medical Assistant Certification.
• 2 years of Medical Assistant experience.
• CPR certification.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care using analytic tools.
• Performs pre-visit planning by chart and data reviews before patient visits.
• Assists with expanded rooming protocol and activates standing orders.
• Documents clinical visits and assists with order entry under supervision.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs registry and quality data review for population outreach.
• Identifies high-risk patients and facilitates complex care management referrals.
• Reviews care team data to improve access and reduce ED visits.
• Cleans and sterilizes instruments and maintains inventory.
• Performs in-house testing following established standards.
• Performs other duties as assigned.
