Job summary

Allegheny Health Network is seeking a Medical Assistant to support population health management through patient outreach and care coordination. You will assist during clinical visits, support self-management coaching, and help maintain office operations under provider supervision.

Responsibilities include closing gaps in care using patient registries, completing pre-visit planning, aiding rooming protocols, performing clinical transcription, and entering orders.

Qualifications

• High School / GED or completion of a Medical Assistant program or LPN license.
• PA LPN licensure or multi-state licensure through eNLC or Medical Assistant Certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care using analytic tools and registries.
• Performs pre-visit planning by chart and data reviews and prepares for patient arrival.
• Assists with expanded rooming protocol and activates standing orders as needed.
• Documents clinical encounters under provider supervision and assists with order entry.
• Coaches patients toward self-management using motivational interviewing and follows up on care plans.
• Performs data review and supports population health goals and care coordination.
