Job summary

Allegheny Health Network is seeking a Medical Assistant/Licensed Practical Nurse to support population health management, assist during clinical visits, and provide self-management coaching to patients.

You will participate in pre-visit planning, review charts in advance, help address care gaps, document encounters under supervision, and support order entry while maintaining HIPAA and safety standards within a collaborative care team.

Qualifications

• Successful completion of a Medical Assistant program or LPN license.
• 2 years of Medical Assistant experience preferred.
• High school diploma or equivalent.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care and uses registries to close gaps.
• Ensures pre-visit planning is completed by chart and data reviews before patient visits.
• Assists the provider in addressing care gaps and activates standing orders during visits.
• Documents elements of the clinical visit under provider supervision and supports order entry.
• Coaches patients using motivational interviewing to support self-management and activation.
• Performs registry and quality data review and outreach to close gaps in care.
• Identifies high-risk patients and coordinates complex care referrals with the care team.
• Maintains instrument sterilization and inventory, stocking treatment areas.
• Performs in-house testing following established standards and policies.
• Performs other duties as assigned or required.

Education

Medical Assistant Program or LPN License
