Job summary

Community Care Plan is hiring a Community Health Coordinator in Sunrise, FL on a full-time basis. The role focuses on engaging with the Director of Community Health to implement and oversee Health Risk Assessments and Expanded Benefit programs under contractual requirements.

The candidate will conduct outreach, assess needs, and track outcomes using CCP software, while supporting internal departments and reporting needs. A hybrid work schedule is expected.

Qualifications

• Associate degree with at least one year of relevant experience.
• Microsoft Office and internet software proficiency.

Responsibilities

• Conduct outbound calls to complete health risk assessments.
• Assess social and clinical needs and refer for follow-up.
• Review Health Risk Assessment responses for Health-Related Needs and eligibility for programs.
• Prepare incentive files and assist with reporting per contract requirements.
• Monitor program utilization and recommend improvements.
• Support outreach to raise awareness of Expanded Benefit programs.
