South Florida Community Care Network LLC in Sunrise, FL is hiring a Community Health Coordinator to support the Director of Community Health in engagement, implementation, and oversight of Health Risk Assessments and expanded benefit programs. The role requires an Associate degree with at least one year of relevant experience and proficiency in Microsoft Office and internet software.

A hybrid work schedule is in place to balance field and office duties.

Responsabilités

Conduct outbound calls to complete health risk assessments and document results.
Assess social and clinical needs and refer to CCP departments for follow-up.
Review Health Risk Assessment responses for Health-Related Social Needs and program suitability.
Track healthy behavior requests and prepare incentive files as scheduled.
Assist with quarterly and ad hoc Healthy Behavior reporting.
Analyze utilization patterns and suggest program changes to boost uptake.
Support outreach and engagement to raise awareness of Expanded Benefit programs.
Monitor programs with dashboards and reporting tools, and collaborate on financial reporting.
