Qualifications

• Education/Experience: Bachelor of Science or Arts in Social Work, Psychology, Human Services, Nursing, Science or Health field with six months' experience in social work or case management.
• Licensure / Certification: CPR Certificate (AHA) and valid Florida Driver’s License.
• Skills / Ability: Verbal and written fluency in English; bilingual in English desirable.

Responsibilities

• Maintain an active caseload of clients needing case management and oversight.
• Monitor clients’ needs and evaluate services for collaborative care among Population Health staff and external partners.
• Develop and promote an effective referral system within CHI to ensure quality care.
• Coordinate with external resources to provide patient services as needed.
• Ensure timely transfer and interdisciplinary collaboration for patient management within 6–8 weeks.
• Document services and assist with funding reports; participate in education and quality assurance activities.
• Attend to patient/community education and, when necessary, conduct home visits.
• Communicate with supervisors and staff; maintain all required licenses and certifications.
• Monitor adherence to treatment and medications; encourage participation in consortium activities.
• Collaborate with care providers to meet treatment objectives and ensure follow-up for missed refills.
