Job summary

CommunityCare HMO Inc. is seeking a Population Health Clinical Assistant to provide administrative and care coordination support to PHM nurses, a clinical pharmacist, social worker, and nurse case managers.

Qualifications

• Strong organizational, customer service and teamwork abilities.
• Ability to manage multiple priorities and communicate respectfully with members and caregivers.
• Knowledge of medical terminology and basic healthcare processes.
• Proficiency with Microsoft Office and electronic documentation systems.
• Ability to maintain confidentiality and perform nonclinical duties under clinical staff direction.
• Successful completion of Health Care Sanctions background check.

Responsibilities

• Receive, enter, and route referrals to PHM team members.
• Contact members to schedule assessments and follow-up calls with staff.
• Organize work queues and notify staff of outstanding needs.
• Request medical records and discharge summaries from providers.
• Coordinate provider appointments, transportation, and community resources.
• Schedule interdisciplinary care meetings and distribute care plans.
• Support pharmacy and social work referrals and track follow-up.
• Prepare and send member/provider correspondence and educational materials.
• Maintain documentation and assist with reports and audits.
• Refer clinical questions promptly to licensed staff.
• Perform other job-related duties as required.
