Qualifikationen

• High school diploma or GED required; and at least four years of experience in quality improvement or population health or a clinical role with direct client care.
• Bachelor’s degree in a related discipline; and at least one year of experience specifically in a population health role.
• Must have experience working in Epic electronic health record system.

Aufgaben

• Use multiple data sources to identify clients due for preventive and chronic care services.
• Generate and distribute client outreach lists for screenings, immunizations, and chronic disease follow-up.
• Conduct outreach via mail, telephone, MyChart, and other methods to support preventive care and follow-up services.
• Place orders for screenings in the electronic health record per protocol.
• Schedule appointments for clients with identified care gaps.
• Track completion of outreach activities and update client status in Epic.
• Develop dashboards and visual reports to support leadership decisions.
