Job summary

Oklahoma City Indian Clinic in Oklahoma City seeks an experienced Epic Healthy Planet Application Analyst to configure, optimize, and support population health workflows within the Epic suite. You will work with care managers, quality teams, and operational leaders to enhance care coordination and preventive care initiatives.

Qualifications

• Bachelor's degree required; equivalency considered with additional experience
• Active Epic certification required and maintained during employment
• 4+ years of healthcare IT, EHR, population health, or care management experience preferred
• Experience configuring Epic Healthy Planet or similar population health tools preferred

Responsibilities

• Configure and maintain patient registries, risk-scoring models, and patient outreach tools within Epic Healthy Planet
• Build care management docs, longitudinal care plans, and roster management configurations
• Maintain metrics for CMS quality measures and public health reporting
• Configure up to 5 Compass Rose program services for care management and social determinants of health
• Collaborate with stakeholders to translate quality reporting into actionable system builds
• Test and validate clinical decision support tools to close care gaps
