Qualifications

HS diploma or GED required; 4 years in quality improvement or population health, or clinical care
Bachelor’s degree in related discipline; 1 year in population health
Experience with Epic EHR system is required
Knowledge of medical terminology and data analysis is preferred

Responsibilities

Identify clients due for preventive and chronic care services using multiple data sources.
Generate and distribute outreach lists for screenings and follow-up.
Conduct outreach via mail, phone, MyChart and other methods to support care.
Review and update client health maintenance information for accuracy.
Collaborate with clinic staff to coordinate site-level outreach efforts.
Develop dashboards and visual reports (Epic Radar, Excel, Power BI) for leaders.
