Job summary

Telos Health Systems is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide geriatric care in Skilled Nursing and Assisted Living settings. The role offers a sign-on bonus and a daytime, two-day workweek chosen by you, with a hybrid on-site/remote charting model.

The PMHNP will lead SNF clinical care, supervise nursing staff, and collaborate with physicians to manage acute and chronic conditions.

Qualifications

• Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Active license and DEA registration required.
• Minimum one year of experience in Skilled Nursing facilities preferred.
• Experience leading care teams in a clinical setting.

Responsibilities

• Act as a leading provider of internal medicine services for SNF residents.
• Collaborate with interdisciplinary teams and physicians on patient care.
• Provide comprehensive health assessments and prescribe treatments.
• Prescribe medications and perform comprehensive physical exams.
• Develop patient management policies and procedures.
• Facilitate referrals and coordinate with other providers.
• Document care and CPT coding accurately.
• Perform monthly stability visits and evaluations for condition changes.
