Telos Health Systems, Inc. is seeking an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to serve as the SNF-ist, providing internal medicine services for geriatric residents in skilled nursing or assisted living settings.

The role includes day-to-day leadership within the facility’s care team and collaboration with physicians and staff. The position offers a $5,000 sign-on bonus and a flexible schedule with two nonconsecutive daytime weekdays.

We are looking for experienced Psychiatric Mental Health Nurse Practitioners, PMHNPs for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.

What You Will Do

• As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services. You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all the facility’s residents.
• You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Prescribe all necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Communicate with attending physicians and/or specialists regarding patient care.
• Be comfortable and proficient with documentation and CPT coding.
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition.
• Advanced Care Planning

What You Will Need

• Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Must have an active license and DEA #
• Previous experience in nursing or other medical fields
• Minimum one-year experience in a Skilled Nursing setting (preferred)
• Ability to build rapport with patients and their loved ones.
• Ability to thrive in a fast-paced environment.
• Excellent written and verbal communication skills
• Strong leadership experience with emphasis on teamwork in a healthcare setting
